I didn't have to get a photo. Just set an appointment, went down to Punta Gorda office, showed my DD 214 (honorable discharge paper) took test on computer, they took my picture like they do at the DMV, did the electronic fingerprints, gave them a check for $117, and had my permit in just over a week. Simple, fast, painless.
I just bought my daughter a gift certificate for a concealed weapons class that includes use of a gun, all ammo, targets, NRA certified instructors, etc. for $50. Once she takes it I'll get her a CC weapon. I'll spring for her permit fee, too. She's worth it!

If you're going in, you shouldn't need to fill out the form, bring a picture or fingerprints, they'll do it there for the same fee. Just make an appointment and show up. Just to make sure, you might ask when you make an appointment if they can do everything there or if you need to bring anything besides your hunter safety card and ID.
