When I was in my 20's I got a Rapala stuck in a tree while snook fishing. You guessed it it came flying back at me with a vengance. I tried removing it from my calf myself, but it just got worse. By the time all was said and done I had to get surgery to get all the hooks removed from my calf.

Worse case scenario for how we fish is either getting hooked when you get spilled in the yak, or sticking yourself while trying to drop the bait. I made the mistake of leaving my wedding ring on, when I went to drop the 15lb bait one of the 14/0 j's not only stuck me the barb got my ring and almost pulled me out of the kayak. Luckily the guy holding the rod was paying enough attention and didn't start trying to set the anchor. There used to be a good youtube vid of a guy who got tipped and ended up with a 20/0 circle in his calf. It took three people bolt cutters a scalpel and 15 minutes to fish that big bastard through lol. It was nasty enough they removed it unfortunately...
